How does customization work? We will make changes to our template and send you a digital proof. For example, you can have any of the text changed to better fit how you run your business. Lines for shipper, pick up and delivery are already on the form. There is a total of 40 load lines but this can be shortened to include any terms of conditions. You will need to supply the terms during the proofing process. The bottom of the form is where the driver and receiver sign.
